Hepatitis C (HCV) Data Model Testing with Existing Data Sets

In February 2017, NACHC convened a meeting of experts in the public health, health information technology, and primary care fields to achieve consensus on the Hepatitis Care Cascade, its data indicators, and an EHR-agnostic data model.

The data model was used to create a monitoring system to track Hepatitis C-positive patients through the Care Cascade from screening to cure. AllianceChicago has partnered with NACHC to pilot test and validate this HCV data model against existing data sets within the data warehouse.

AllianceChicago will implement the EHR-agnostic HCV data model within the current health information technology infrastructure to ascertain the usability and feasibility of the data model for broader and national use.
